A Case of Acute Intermittent Porphyria Associated with Syndrome of Inappropriate Secretion of Antidiuretic Hormone

Abstract

|
A 34-year-old female patient was admitted due to generalized weakness and intermittent abdominal pain. On admission, showed geeralized seizure and marked hyponatremia. And then she was confirmed as acute intermittent porphyria by Wason-Schwartz
test and
assay of ¥ä-ALA and porphobilinogen on 24 hours collected urine.
SIADH was diagnosed as hyponatremia, high urinary sodium loss, higher urine osmolality than that of serum, normal thyroid function and normal ACTH and cortisol levels. She was treated with hypertonic saline and glucose solution, and then,
completely
recovereed.
We report a case of acute intermittent porphyia associated with SIADH and brief review of literature.
